The purpose of the 2012 CDPM Provincial Conference is to provide health care practitioners, policy makers, administrators, organizations, and individuals committed to prevention and living well with chronic disease an opportunity to share ideas, practices, and initiatives.
This Conference is an opportunity to listen to others, to learn about the many successes in the field, and to work more closely together with your colleagues, many of whom are presenting their work, to further optimize the care of patients with chronic diseases in Saskatchewan.
Physician Continuing Education Credits will be available. A certificate of attendance will be provided.
